  • A referral letter from a registered doctor or specialist so you can claim your Medicare rebate
Note: You don't require a referral to see Dr Sara Safari

NOTE:

If you've seen a physiotherapist, please bring a progress letter.

If you've had surgery elsewhere, please bring a copy of your operation report

If you have questions, it can be useful to note them down to help ensure we cover everything you need to discuss.

WorkCover and TAC Patients

You'll need to pay for consultations on the day and send your claim to the applicable party. Once any required surgery is approved, we will bill your insurer or agency directly.

When to arrive

Please arrive at least 10–15 minutes before your appointment. Arriving early gives you time to relax and complete any paperwork.

NOTE: We send you a copy of our New Patient Registration Form with your letter confirming your appointment.

FAQ for all our patients

How and when do I pay?

You need to pay for your consultation on the day. We accept cash, EFTPOS, and credit card payments.

We can either send your claim directly to Medicare or give you a receipt so that you can claim your rebate.

How long is my appointment?

Mr Moaveni, Dr Safari, and Mr Wan Zhang’s appointment times vary.  Please check this with our receptionist when you make your booking. If you feel you’ll need more time, she can offer you a longer appointment.

We do our best to run on time.

What if I need to cancel?

Please give us at least 24–48 hours' notice so we can offer your place to someone on our waiting list. If you give us less than 24 hours' notice, you may need to pay a cancellation fee. Please note that Medicare rebates don't apply to cancellation fees.

Can I claim a private health insurance rebate for my appointments?

No. Private health insurance rebates only apply to hospital admissions and in-patient treatment.

Can I book a Telehealth appointment?

Yes. You can book a Telehealth consult by telephone or video. If you prefer a video appointment, we'll arrange a link with you.
